高原地区占我国国土面积四分之一,战略地位重要。高能及低能爆炸物产生的超音速、亚音速冲击波等方式致伤,其特有的气候、地理、交通及医疗水平下,对救治时效性要求高、难度大[1,2]。高原环境下军事冲突、恐怖袭击、生产及生活事故等致伤的人群不同,发生爆炸的海拔不同,造成的损伤严重程度不相同。一般海拔越高,病死率及救治难度越高,亟需从军事医疗系统向民间医疗系统的转变和融合,规范高原特殊环境下的分诊和早期救治方案。本文对国内外最新循证医学证据从伤情评估、气道管理及呼吸功能维持3个方面,提出高原爆炸性肺损伤(HAELI)的院前和早期救治策略,为高原环境下爆炸伤的救治提供参考。
