急性脑梗死并发吸入性肺炎的危险因素及预后分析

目的

分析急性脑梗死并发吸入性肺炎的危险因素及预测预后的意义。

方法

选择2021年1月至2023年11月我院收治的96例急性脑梗死患者为对象,并发吸入性肺炎41例为观察组,未并发肺炎55例为对照组。收集患者临床资料,采用Logistic回归分析急性脑梗死并发吸入性肺炎的危险因素,分析预后。

结果

多因素Logistic回归分析显示,年龄(OR=1.119,95%CI:1.029~1.217,P=0.009)、TACI与POCI分型(OR=0.026,95%CI:0.001~1.065,P=0.048)、NIHSS评分(OR=9.231,95%CI:4.110~20.731,P=0.000)及C反应蛋白(C-reactive protein, CRP)水平(OR=2.660,95%CI:1.215~5.824,P=0.014)是急性脑梗死并发吸入性肺炎的危险因素;NIHSS评分(OR=8.109,95%CI:1.788~36.773,P=0.007)、CRP(OR=1.434,95%CI:1.079~1.905,P=0.013)、中性粒细胞-淋巴细胞比值(neutrophil-lymphocyte ratio, NLR)(OR=3.248,95%CI:1.103~9.560,P=0.032)、肌酐水平(OR=1.304,95%CI:1.040~1.636,P=0.022)是急性脑梗死并发吸入性肺炎预后不佳的危险因素;CRP预测急性脑梗死并发吸入性肺炎预后不佳风险AUC为0.922,敏感性为83.33%,特异性为93.98%。

结论

年龄、TACI与POCI分型、NIHSS评分及CRP水平是急性脑梗死并发吸入性肺炎的危险因素,NIHSS评分、CRP、NLR、肌酐水平等对预测急性脑梗死并发吸入性肺炎预后具有意义。
