摘要

目的

分析远程监控调节家用无创呼吸机治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭患者的临床疗效。

方法

招募70例COPD患者合并Ⅱ型呼吸衰竭的患者,最终入组60例作为研究对象。受试者通过随机数字法分为观察组和对照组,每组30例。观察组辅以远程监控调节家用无创呼吸机进行无创正压通气治疗,对照组给予持续低流量吸氧治疗。两组均予以常规治疗。两组受试者随访1年。比较两组生命体征、动脉血气分析、血清炎性因子、生活质量评分及急性加重次数。

结果

CAT评分、mMRC评分、6分钟步行距离(6MWD)、急性加重次数较对照组均有改善(P<0.05)。此外,治疗后两组患者心血氧饱和度(SaO2)、动脉血氧分压(PaO2)、二氧化碳分压(PaCO2)、降钙素原(PCT)、C反应蛋白(CRP)、白介素6(IL-6)、白介素8(IL-8)、肿瘤坏死因子(TNF-α)较对照组均有改善(P<0.05)。

结论

远程监控调节家用无创呼吸机促进患者的康复,提高了患者的生活质量。

Abstract

Objective

To evaluate the effect of remote monitoring and adjusting non-invasive ventilator to improve the symptoms of chronic obstructive pulmonary disease(COPD)patients with type Ⅱ respiratory failure.

Method

All of 70 cases COPD patient with type Ⅱ respiratory failure recruit for this trial and final 60 subjects enrolled in this study. These subjects randomly allocated (1︰1) in the observation group and control group by the random number method. The subjects in the observation group received remote monitoring and adjusting non-invasive ventilator, and the control group received continuous low flow oxygen inhalation. Furthermore, all subjects received COPD therapy. The two groups of subjects were followed up for 1 year and vital signs, arterial blood gas analysis, serum inflammatory factors, quality of life score, and the number of acute exacerbations had been evaluated.

Results

The CAT score, mMRC score, 6 MWD improved after subjects received remote monitoring and adjusting non-invasive ventilator therapy, compared to the control group (P<0.05). Moreover, the level of SaO2, PaO2, PCT, IL-6, IL-8 and TNF-α decreased in the observation group, compared to the control group (P<0.05).

Conclusion

Remote monitoring and adjusting non-invasive ventilator effectively improved the symptoms of COPD patients with type Ⅱ respiratory failure, promoted pulmonary rehabilitation of COPD patients.
