COPD并发低钠血症的危险因素分析

慢性阻塞性肺疾病(chronic obstructive pulmonary disease, COPD)长期反复肺部慢性炎症,常伴慢性呼吸衰竭、低氧等,引起胃肠道淤血、食欲下降,钠摄入不足,发生呕吐、腹泻等,钠排出增多,Na泵功能失调,引起低钠血症[1,2]。低钠血症是临床常见电解质紊乱,是多种潜在疾病严重程度和预后重要指标,低钠血症死亡风险增加1.3倍[3]。低钠血症是COPD最常见电解质紊乱,发生率25.8%~40.5%,延长住院时间,增加发展为AECOPD几率,是长期治疗无效的主要原因[4]。目前女性、年龄、糖尿病及利尿剂使用是低钠血症发生的危险因素,无特异性[5]。本文对我院收治的59例COPD老年患者低钠血症发生的影响因素进行分析,报道如下。
