老年髋部骨折患者并发坠积性肺炎防治进展

全球每年大约发生150万起髋部骨折,预计到2050年将增加到630万[1]。髋关节骨折的病死率非常高,骨折后1年内高达8.4%~36%[2]。老年人髋骨骨折病死率相对年轻人高。术后并发症是导致髋部骨折高病死率的主要原因,其中术后坠积性肺炎是髋部骨折患者最常见和最重要的并发症之一[3]。临床实践中即使给予标准治疗和严密监测,坠积性肺炎(hypostatic pneumonia)患者仍有较高的病死率和不理想的结局,14%的髋部骨折患者在30 d死亡,相比之下,没有坠积性肺炎的患者只有1.7%[4]。本文对老年髋关节骨折患者术后并发坠积性肺炎的预防研究进展作一综述,以延缓老年髋关节骨折患者的术后生存期,提高生存质量。
