Abstract:

Objective

To investigate the clinical effect of ipratropium bromide combined with non-invasive ventilation therapy in patients with chronic obstructive pulmonary disease (COPD)and respiratory failure, in order to provide a new idea for the clinical treatment of COPD patients with respiratory failure.

Methods

All 90 patients of COPD with respiratory failure were randomly divided into control group and research group from February 2015 to December 2016 in Department of Respiratory, Jiangsu Jiangyin People′s Hospital. Each group had 45 cases. The two groups of patients were treated with anti-inflammation, anti-asthma, reducing phlegm, oxygen therapy and other conventional treatment methods. The control group was treated with non-invasive ventilation therapy, and the research group was treated with ipratropium bromide on the basis of the control group (aerosol inhalation with ipratropium bromide 500 μg and NS 1 ml tid). After 7 days′treatment, the blood gas analysis, lung function, CAT score, Borg score of two groups before and after treatment were compared.

Results

The PaO2, FEV1, FEV1/FVC of two groups after treatment increased significantly, and the PaCO2 decreased significantly, the difference was statistically significant (P<0.05). Comparing with control group after treatment, the PaO2, FEV1, FEV1/FVC of research group after treatment increased obviously, and the PaCO2 decreased obviously, the difference was statistically significant (P<0.05). CAT score, Borg score of two groups after treatment decreased significantly, the difference was statistically significant (P<0.05). Comparing with control group after treatment, CAT score, Borg score of research group after treatment decreased obviously, the difference was statistically significant (P<0.05).

Conclusions

Isopropyl bromide combined with noninvasive ventilator plays a synergistic role in the treatment of COPD combined respiratory failure. It can effectively alleviate the symptoms of airflow obstruction, improve the lung function, the clinical effect and quality of life of patients. It is worthy of clinical applications.
