Abstract:

Objective

To analyze the correlation between serum lipid transporter and the severity of chronic obstructive pulmonary disease (COPD) patients.

Methods

A total of 68 COPD patients admitted to our hospital from January 2017 to January 2019 were selected and divided into stable group (COPD stable period, 38 cases) and recombination group (COPD acute exacerbation period, 30 cases) according to the degree of disease; During the same period, 35 cases of healthy physical examination were the control group. The white blood cell count (WBC), C-reactive protein (CRP), and the percentage of neutrophils (NEU%) were compared between the two groups, and the serum amyloid A (SAA), tumor necrosis factor-α (TNF-α), alveolar Surfactant protein D (SP-D), lipid transporter 1 (LCN-1) and lipid transporter 2 (LCN-2), analyze SAA, TNF-α, SP-D, LCN-1, LCN-2 Correlation analysis with the severity of COPD patients, using receiver operating characteristic (ROC) curve to analyze the diagnostic value of SAA, TNF-α, SP-D, LCN-1, LCN-2 in the acute exacerbation of COPD.

Results

The smoking history of the recombination group was higher than that of the stable group(P<0.05); the levels of FEV1/FVC and PaO2 were lower than those of the stable group (P<0.05); there was no significant difference in BMI index between the three groups (P>0.05). The recombined CRP, WBC, and NEU% were all higher than the stable group (P<0.05). The levels of recombinant TNF-α, SP-D, SAA, LCN-1 and LCN-2 were higher than those of the stable group (P<0.05). According to Pearson test, SAA and TNF-α are highly positively correlated with the severity of COPD patients (P<0.05), SP-D, LCN-1, LCN-2 are positively correlated with the severity of COPD patients (P<0.05). After ROC curve analysis, it is found that SAA, TNF-α, SP-D, LCN-1, LCN-2 have certain diagnostic value in the acute exacerbation of COPD, and the areas under the curve are 0.768, 0.739, 0.696, 0.840, 0.766, respectively.

Conclusion

COPD is not only related to SAA, TNF-α, and SP-D, but also related to LCN-1 and LCN-2, which can reflect the severity of the patient′s condition and has a certain diagnostic value in the acute exacerbation of COPD.
