特发性肺纤维化患者诱导痰中SDF-1的表达与高分辨CT表现的相关性研究

目的

探讨特发性肺纤维化(IPF)患者血清和诱导痰中基质细胞衍生因子-1(SDF-1)的表达水平与肺功能及胸部高分辨CT表现的相关性。

方法

选取IPF患者32例及健康体检者30例,收集血清和诱导痰标本并检测SDF-1表达水平,IPF患者行肺功能和胸部高分辨CT检查,并对结果作相关分析。

结果

IPF患者的诱导痰中SDF-1的表达水平为(705.21±11.71)ng/ml,高于健康体检组的(252.82±10.62)ng/ml,P<0.05差异有统计学意义。血清中SDF-1的表达水平(5.29±0.13)ng/ml高于健康体检组的(4.93±0.14)ng/ml,但P>0.05,没有统计学意义。IPF患者肺功能均有限制性通气功能障碍和弥散功能障碍;影像学表现为程度不一的磨玻璃影、网格影和蜂窝肺,HRCT评分与诱导痰中的SDF-1表达水平呈正相关(r=0.287,P<0.05)。

结论

SDF-1在IPF患者诱导痰中表达明显升高,并与患者病情严重程度(胸部高分辨CT表现)呈正相关,对IPF诊断和治疗具有潜在的参考价值。

Objective

To evaluate concentrates of stromal cell-derived factor in serum and induced sputum supernatants from idiopathic pulmonary fibrosis(IPF) patients, investigate the relationship between the stromal cell-derived factor-1 (SDF-1) level and the high-resolution computed tomography parameters in patients and explore the role of SDF-1 in the diagnosis and treatment of IPF.

Methods

Serum and induced sputum samples were collected from 30 healthy controls and 32 patients with IPF. The concentrations of SDF-1 in serum and induced sputum were measured by ELISA. Lung function and chest high-resolution CT were executed in patients with IPF. Analysis of correlation was performed on these results.

Results

The concentration of SDF-1 in induced sputum was significantly increased in IPF group(705.21±11.71)ng/ml compared with healthy group(252.82±10.62)ng/ml(P<0.05). The level of SDF-1 in serum was also elevated in IPF patients(5.29±0.13)ng/ml compared with healthy group(4.93±0.14)ng/ml, but there was no statistically significant difference (P>0.05). All patients with IPF have restrictive ventilation dysfunction and diffusion dysfunction. There was a positive relationship between induced sputum SDF-1 level and HRCT score(r=0.287, P<0.01).

Conclusions

The expression of SDF-1 in induced sputum was significantly increased, and associated positively with severe degree (chest high-resolution CT manifestations) of patients with IPF. SDF-1 may play an important role in the pathogenesis of IPF, therefor serve as a potent testing index of the diagnosis and treatment of IPF.
