肺内畸胎瘤临床特点及文献复习

目的

综合分析54例肺内畸胎瘤(IPT)患者的临床资料，以提高肺内畸胎瘤的诊断率。

方法

在PubMed、万方数据知识服务平台、中国期刊网全文数据库中检索2000年至2016年报道诊断为IPT的病例52例，并结合我院2例IPT综合分析其临床特点及误诊原因。

结果

IPT好发于男性不吸烟的青年患者，病变部分好发于左上肺(55.6%)，临床症状以咳嗽为主(92.6%)，其中尤以咳毛发样物质、豆腐渣样物质为特征性的临床表现，影像学均可见肿块、结节影(100%)，支气管镜下以见毛发样物质为主(40%)。肺内畸胎瘤临床表现、影像学缺乏特异性，病情隐匿，故常导致误诊为肺结核(22.2%)、肺脓肿(14.8%)、侵袭性肺曲菌病(3.7%)、肺癌(3.7%)等疾病，本组研究中30例(55.6%)患者被误诊。

结论

IPT临床表现特异性少见，常被误诊其他疾病。临床上见咳毛发样物质或豆腐渣样物质及支气管镜下见毛发样物质时，应高度警惕肺内畸胎瘤的可能性，尽早行病理组织学检查以明确诊断。

Objective

To discuss the clinical characteristics, pathological features, and reasons for under-diagnosis of 54 intrapulmonary teratoma (IPT) patients.

Methods

It was searched that the 52 IPT patients in PubMed, WANFANG DATA and Chinese National infrastructure(CNKI) from 2000 to 2016 and combing with 2 cases diagnosed with IPT in Xiangya Hospital of Central South University to analyze the clinical characteristics and reason for under-diagnosis.

Results

The majority of IPT patients occured in young male adults without smoking which located at the left upper lobes(55.6%). The major clinical characteristics of IPT were cough (92.6%), hemoptysis (72.3%) and expectoration(57.4%), especially for cough hair sample material, jerry-built material. Besides, imaging studies always presented the mass lesion(100%). The hair distinguishing features can be found through bronchoscopic testing(40%). The IPT was easily under-estimated because of sharing the same clinical feature with invasive pulmonary aspergillosis, lung cancer et al. Among our study, 30 patients (55.6%) had been mainly misdiagnosed as tuberculosis(22.2%), pulmonary abscess(14.8%), invasive pulmonary aspergillosis(3.7%), lung cancer(3.7%).

Conclusions

This study demonstrates that the clinical doctor should be pay more attention to patients who cough the hair or found the hair substance under bronchoscopic testing. The systematically histologic biopsy testing is necessary to improve the diagnosis.
