经鼻高流量湿化氧疗治疗AECOPD伴Ⅱ型呼吸衰竭的临床分析

目的

分析慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease, AECOPD)伴Ⅱ型呼吸衰竭予以经鼻高流量湿化氧疗(high flow nasal cannula, HFNC)的临床疗效。

方法

选择2022年1月至2023年12月我院收治的71例AECOPD伴Ⅱ型呼吸衰竭患者为对象,予以无创正压通气(non-invasive positive pressure ventilation, NPPV)37例为对照组,予以HFNC 34例为观察组。分析两组血气水平、呼吸循环指标、舒适度及临床结局情况。

结果

观察组治疗后较对照组动脉血氧分压(arterial partial oxygen pressure, PaO2)[(73.26±4.11)vs. (72.32±3.98)]mmHg、平均动脉压[(78.45±8.65) vs. (78.02±8.52)]mmHg、动脉血二氧化碳分压(arterial partial pressure of carbon dioxide, PaCO2)[(43.65±3.68) vs. (44.98±4.02)]mmHg、乳酸(lactic acid, Lac)[(2.45±0.44) vs. (2.41±0.43)]mmol/L、心率[(78.25±11.32) vs. (77.25±12.64)]次/min、呼吸频率[(20.12±1.25)vs. (20.37±1.35)]次/min(P>0.05);观察组舒适度评分(6.73±1.25)分高于对照组(5.68±1.89)分(t=2.782,P<0.05)。两组ICU住院时间、气管插管率及病死率差异无统计学意义(P>0.05)。

结论

HFNC改善AECOPD伴Ⅱ型呼吸衰竭血气水平,提高舒适度,具有临床意义。
